While working with Foundation 2, information regarding your medical history, treatment, social history and other issues including payment for services may be created or received by us. Information which can be used to identify you relating to your medical care or payment for your medical care is protected by state and federal law (“Protected Health Information or Protected Information”).

Of the information we learn about you from your visit to “https://www.foundation2.org,” we collect only the following: the date and time of your visit, the name of the file or the words you searched, and the browser used to access our site. This information is used to measure the number of visitors to the various sections of our site and identify system performance or problem areas. We also use this information to help us improve our site. We do not give, share, sell or transfer any personal information to a third party.

If you submit a question or comment to the website (such as in our “Ask a Counselor” section) we will not include identifying information. We do not identify publicly who sends questions or comments to our web site. We will not obtain information that will allow us to personally identify you when you visit our site, unless you chose to provide such information to us.
